To process your claim efficiently, please ensure the claim form is filled out in its entirety and that you provide the following:

• Your policy number (found on your Certificate of Insurance).

• Details of the defect, including key dates and how the issue occurred.

• Supporting documents such as relevant photos or reports (if applicable).
